Eligibility Requirements

Caregiver or Chore Services

Applicant must be (whichever is applicable):
• Enrolled member of the Puyallup Tribe of Indians;
• Elder: 55 years of age or older whose ability to perform the normal activities for daily living or to provide for his or her own protection is impaired due to a physical disability or kidney dialysis; or

Companionship Services

Applicant must be (whichever is applicable):

  • Enrolled member of the Puyallup Tribe of Indians;
  • Elder who is 55 years of age or older and has a need for supportive companionship; or

Transportation Services

Applicant must be:

  • Enrolled member of the Puyallup Tribe of Indians;
  • Elder who is 55 years of age or older and has a need for transportation services; or

Services Provided

Caregiver Services (include but not limited to):

  • Personal care: bathing, personal hygiene, dressing, grooming, toileting;
  • Meal planning and preparation;
  • Housekeeping: linen changing, laundry etc;
  • Exercise;
  • Transfers and positioning;
  • Transportation;
  • Medication assistance, and
  • Welfare checks.
  • Chore Services (include but not limited to):
  • Laundry and change linens;
  • Light housekeeping: clean kitchen, wash dishes, take out garbage, sweep, mop, vacuum floors, clean bathrooms;
  • Medical reminders and delivery;
  • Meal planning and preparation;
  • Shopping for food or other essential home items;
  • Transportation (see full policy); and/or
  • Mail retrieval and drop off.

Companionship Services (include but not limited to):

  • Chores services as defined above and several hours after chore services are completed to:
    • Complete additional unanticipated tasks; and/or
    • Provide psychological and social support.
