Eligibility Requirements

Applicant must be:

  • Enrolled member of a Federally Recognized Tribe;
  • Spouse to enrolled Puyallup Tribal Member

Application Process

Applicant must:

  • Fill out the Application for Services
  • Provide documentation of enrollment, i.e., copy of Tribal identification card;
